I have a premium account but I am still getting adds. This has never happens before it only started recently, after I took a two month break from anime to focus on school. I came back today and I was getting adds, even though I still have premium. Does anyone know why?

Your profile page says you are not a premium member. Are you seeing another indicator elsewhere that you are still premium? Is it possible you have another account which is actually the premium one?

I suggest contacting customer support directly via the/contact form to get help sorting it out.

If I go to my phone settings and look at subscriptions, Crunchyroll premium subscription is one of them. Also if I try to buy a subscription through the app, it states that I already have a subscription so I cannot buy one.

Signing up through one of the apps has been known in the past to create a new account with the associated email address (or gamer tag) of the device in some cases.
Try logging in to the site with the email address associated with your phone as the log in name.

If that doesn't work, or does and you need to merge the accounts, /contact support like lorreen suggested.

It seems like your recent transaction through iTunes failed.
I would suggest checking your iTunes account or contact Apple Support for a solution.
