Fred - Although common, I got it from a stephen king book called Roadwork where the main character has a voice in his head named Fred.
Grim - From my old laserforce days I had the alias Grimreaper.
If I made a character for the name it would be some guy related to the grim reaper. Kinda like the chick in Discworld.
