I'd have to say Boku no Hatsukoi wo Kimi ni Sasagu
AKA - Boku Kimi.
I haven't finished it but it's so sad. When they were little they promised that they'd get married when they were twenty, then the boy finds out his hearts all messed up and he wont live to be 20.v___v
