MPD Psycho. Hands down, the most messed up manga I have ever read. The tv series (what few episodes I've seen anyway,) is pretty messed up too, although it's not surprising considering that it's directed by Takashi Miike.

Goth.. it.. it was horrible!! It was totally psychotic (not so much weird, just gory) and has forever messed up my pure mind. To others it might not seem that bad, but to me.. it was just beyond what my poor little brain could take...lol. It was also my first time reading a manga like it, so it truly shocked me. |

Ok, I am surprised nobody has mentioned Uzumaki! It is the weirdest, creepiest, and one of the most disgusting horror mangas I have ever read. And I have read some messed up stuff including Hideshi Hino's The Gallery.

sorry for the second post, but i thought it was worth mentioning Narutaru.
god, this manga is messed up. there's gore, child sex/pregnancy/rape (this one girl gets raped with a needle, for gods sakes), implied incest, torture, and weird-ass looking aliens that tear people about.
i think its an anime now though, but the manga is just hardcore messed up. Freaked me out.

A really good one is After School Nightmare! I really enjoyed it but was sad that it's short. OH, and Rabbit Doubt is good too! I read a lot of manga of these sort of genres so i can give you a better list later.
