Things that piss you off in games:
1) Quick time events.
2) Multiplayer cheaters and those that disconnect when they're losing.
3) Voice dubbing on Japanese games. Please give me the option for the original audio with English subs.
4) Nolan North
5) Patches. Test your games before you release them (Fallout: New Vegas).

Ahh, this. I really like Phantasy Star Universe and Ninety-nine Nights, but the English dubs on both games are pretty horrible.
Bad cutscenes (another thing 99 Nights is bad for) don't really piss me off, but they are disappointing when I'm playing a game that is otherwise a lot of fun.
Only other thing I can think of right now is something that happens to me all the time in RPGs, when you work your ass off to buy or make some great new weapon, then you find it in a chest 10-15 minutes later.

In single player games I HATE bosses you have to lose against. (sometimes I restart before i see game over.. So sometimes it takes me awhile to realize)

I hate playing S4 League and your outnumbered, barely winning, and some dumbass starts bitching about your ping then your teammates start getting on you about it. And its like "what the hell do you want me to do about it? We're winning and they asses are hacking... appreciate my high ass ping bitch!"
That and when you empty the clip into your opponent not missing once and they still manage to run you down and kill you with a bat/sword while you reload to try it again

OMG I love s4! Im Pro at there! But quitted a couples of month ago. Cuz' its getting boring and many hackers!
I also remembered when Im still noob! when your team lose they blame you for being noob!
Also I have many friends who played s4, but also HACKS!
WE know that It's sucks when were playing with a hacker! its kinda unfair tho.!

When there are no hard bosses.
When there is only one hard level, and the rest suck.
When you get to the last level and think, wow, the other level were harder then this. :|
When there are no characters that you like.
When the story sucks.
When there are only 5 or 6 levels.
When the game just dosn't make a bit of sense.
Dramatic scenes that really aren't dramatic.
Glitches at the wrong time.
When you die, really annoying music comes on. (God knows that gets annoying after a while of dying.)
Sooo...Yeah. :|

When there's no saving points and if you die/lose far into the game, you have to start all over from the beginning.
