Medium doesn't necessarily mean the song is medium difficulty.
If things aren't syncing up check your A/V delay(adjust at setup) or buffer size(should be default setting). Whether songs are under fretted/over fretted is another issue, but what's there should sync up.

If you look to the left once you click the link you'll see a "switch view" option, which should make the descriptions visible if they are not. The descriptions tell you the name of the song that the update belongs to. After you download just copy and paste, replacing the old notes.mid etc. If you're looking for a recent update make sure to sort by date to arrange the list by "latest added".
Add "lyrics=True" without the quotes to song.ini and save if you are adding lyrics. Of course you also need a mod that enables lyrics support otherwise they won't show up.
