The Second Button is considered to be a High School Memento. It is somewhat a tradition in Japanese schools that girls got to have the 2nd button of the guy they like or the one who they considered to have spend the most memorable time with. Although that custom is somewhat rare this days, it is always presented on anime's or dramas to add a catch.

If there are multiple girls in love with a certain guy, the girl who gets the 2nd button or is given it is considered the winner in his heart. It's more like an indirect confession.

I'm referring to the 2nd button on his shirt/uniform. They did this so that it would only have little effect on the uniform (it would be revealing if it was the first button on his shirt and dangling if it was the last). Either the girl grabs it (usually the girl asks for it first and if approved grabbed it by the mouth and cuts it by her teeth) or the guy presents it to the girl he likes(or spent a good memory together) and if she accepts it, it's like they became lovers already.

ok, i was watching Itazura na Kiss and on the 7th episode everyone kept asking Irie for his 2nd button. i was wondering why they do that. i never understood it, and i have been watching anime since forever and when ever they show it happening. i never get it. like is it supposed to be a sign of affection or something, and like how does that like work? i find it odd. like i just wanna know.

*i just read some of the replys, and GOSH why are people being such jerks! like seriously, every time i ask a question, people go crazy on me. like chill. every time i asked a person they didn't know the answer.

thanxs to anyone who is ACTUALLY being nice and helping me. it really helped clear things up. and to the others. "Im sorry for being kinda new at this and not know i am not allowed to post a question. my bad. seriously harsh much.

I don't think this is such a stupid thread... It's interesting. I've been watching anime since I was 3 and I've never figured out what the button thing was (I could have guessed).

yup, that's also one explanation. There is a story behind that one. A girl on a graduation day confessed to a guy but the guy turned her down. The girl cryingly ask for the guy's second button. When ask why, she only said 'This, is enough for me. If I just could hold on to one thing closer to your heart. This will be my most important memory."
