I will say Higurashi no nako koro ni and Elfen Lied are the main two series for creepiest for me.

Craziest anime I have seen: Le Portrait de Petit Cossette.

Crazy because of its unsual narrative and cinemagraphy (trying to distant the viewer, with a purpose of course), and occasional random graphics (symbolic I think?) that are not part of the story popping in. I like its mood. But I don't think the script (plot twist comes in ep. 3, a bit late) and drawing style stand out much. And the narrative sometimes makes it a bit difficult to follow.

Extreme graphic violence (sometime it is even pointless, violence for the sake of violence, probably to seek attention) is not cool. It's been used to death and is boring. The most crazy anime should be those that challenge conventions and do something that others haven't done before. "Moe" girls, ecchi and fan service are not cool either. The most creepy anime should be those that use creative techniques instead of violence and senseless unrealistic "psycho" behaviour to create the scariness and creepiness.

As to the Higurashi one everybody's been talking about, its graphic style is just not pretty enough to make me watch. So I wouldn't bother.

Hell Sing OVA's has some pretty crazy scenes

meh, Pet shop of Horrors was pretty crazt and creepy, old fashion of course. Gantz was crazy, considering people die at a place then get sent to a room, to kill aliens, loads of blood and gore, some nudity.

Ghost Hunt was a little bit scary. ^^
