She's dead... 
She was the Nine Tails jinchuuriki... Kushina wanted Minato to seal the Fox back into her so that she will kill the Fox with her death... But then (when Naruto was born), Madara, freed the Nine Tails... Some people say that, she died when she was giving birth to Naruto... But some people say that she died another time.... I really don't know. Kushina doesn't does not appear that regularly...

she died when the kyubi was extracted from her and unleashed on Konoha truthfully, but due to her longivity she didn't croak till she and Minato sealed the Kyubi into Naruto.

Kushina and Minato even protected Naruto when Kyuubi realised what was happening and tried to kill him. They let their own bodies get impaled so Naruto was unharmed.

Don't forget the Eight Trigrams seal as well, that was used to seal Kyuubi in Naruto, as well as parts of Minato and Kushina so they could help him out later on.
