Clannad knocked Grave of the Fireflies off the top of my list. (Grave is so depressing I wasn't ever able to watch it again) but my two favorites now days are these: The most touching single episode would the the 12 episode of "And Yet The Town Moves". When Hotari realizes that there isn't going to be a "Next Time" it's absolutely heartbreaking. The most touching series is Anohana: The Flower We Saw That Day. I'm a big, tough guy but I weep every time I watch those.

Puella Magi Madoka Magica (ESPECIALLY the heartbreaking finale).
Princess Tutu (After all, the series involves a girl who collects feelings to return to a person who has lost them).
Koi Kaze (A series bout incest that was actually handled very realistically and it WAS really touching and beautifully done).
Sailor Moon (So many times this series touches you).
