membership cards application form ^_^

Anyways, I'll go with request any character system.. you may request the character you want to have as membership cards.. as of now I have 2 versions of haruhi membership cards for sample.. if you're contented with haruhi then I'll give it to you ^_^ anyways..

note: when 2 users request for the same characters, I'll be creating two types.. like what I did with haruhi.. and also, if you want me to put some images that you have you may do so..
