It's annoying. I hate when people say, "No offense," because we all know it's going to offend. May as well stick to what you say shamelessly or keep your fucking mouth shut. I try to not say it, unless it's to a complete stranger.

Edit: Actually, I can't think up of any scenarios where I would have to say it to a complete stranger, so nevermind.

Yeah I agree, if you say 'no offence' might as well not say it at all.

The person is just stating his opinion, if it offends you, then it's your problem
at least saying "No offence", he's trying to show no harm, and hes not being totally direct

If it's sarcasm. Probably it'll irritate me.

Well, if they really mean it, I don't really mind that much.

But if they insulted me and said 'no offence' in a sarcastic way, I would retaliate and insult them back and say no offence in-a-not-so-honest-way back

It's a way to gesture politeness. If you think that it's still offensive, then the problem is you yourself.
They're already slowing things down, yet you still think that it's advisable to act like a nuisance with the current conversation.
It's called being Ignorant.

just accept the fact you are going to be offended.
if the words are thrown irrationally, you can kick them back.

no offense and all but saying no offense should not offend you because when people imply no offense there really isn't meaning behind saying it because why would they take the time to say no offense if they wanted to offend you because when people want to offend others they don't say no offense but when they mean no offense to you they'll say no offense.

just seeing how many times i could use offend/offense in a semi run on sentence =P
