I will try an answer a few of the questions I've read here so far. I am by no means the final "answer-giver" , but I do have a deep interest in the horror genre.

A: Yōkai & Yuki-Onna are demons in Japanese folklore, and recognized pretty much universally in Japan and their images have been sort of immortalized into the popular horror culture and used and reused over and over again, (not just by Japanese directors, but by almost all in the genre) but not specifically stated so by directors. A: The answer to this one is debatable based on your point of view. It's is a common misconception or preconception from the East that the West (read Americas) are either used to the violence because of our television programming ( CSI , NCIS , Wife Swap (hehehe) etc ...) and news broadcasts are literally filled with violent imagery. There is an old saying in the publishing business, "If it bleeds, it leads" which just alludes to the fact that most Westerners are simply drawn to other peoples adversary. Which in part is completely true. The other side of the coin is that most Westerners, are not privy to the day to day atrocities some Eastern cultures have to face daily ( Tsunami's , Earthquakes, Volcanoes etc) and as such are less exposed to some of the more horrible ways to die. Knowing these two fairly indisputable facts, most "gore" directors will hone in on the genre, simply because the Western audience eats it up whether it is by curiosity or simply if I may quote a friend from Massachusetts "looks effing wicked sick". So why is this? Once again conjecture rules the day, but based off the fact that 100% of that 85% in which the minority eats it first, have been American made films, I would say that the minority served as an allegory for intolerance to others. Not only this but also due to the fact that most horror films have an underlying theme in them ( which I will cover in the next question ) they tend to try to get across the themes in a certain dictated order ( see: Scream ). 2: They can not get the keys because of how the mind works in a fear scenario. Allow me to quickly elaborate: When faced with a potentially life threatening scenario, the human evolutionary conditioning is either Fight or Flight. It has been proven our perception of time slows down when faced with these situations. Thus in the time it takes them to find their keys say normally 10 seconds, overall the actual time should be 30% faster. Giving the killer an extra 3-4 seconds to get to them. Oh and if they do get the keys, the car never starts anyway :) 3: Otherwise there would be no sequel or final "gotcha" moment! Q: A: First I will refer you to the answer above in 1: and I stated I would return to it and this is exactly the venue! Okay! Every horror film has an underlying theme E.G. Pre-Marital Sex is bad / Drugs are bad / Cheating is bad/Virginity & Chastity are good. You will find these in almost EVERY horror movie. I highly recommend Behind the Mask: The Rise of Leslie Vernon. It is a great movie to fill you in on all the little underlying things in a Serial Killer / Preternatural / Supernatural killer film.
