Cold always....or hot (quick microwave) when it's cold weather.
BUT never warm....feels/tastes gross

Cold milk...
I've never even had hot milk, but then again, I don't drink milk frequently. Not anymore. I loved it as a kid, but now, I mainly use it for cooking stuff, and don't drink it as much. Makes my breath smell.

Both, although I usually add a little chai, milo, or hot chocolate mix into hot milk.
