Kefka was most definitely the craziest psycho ever.
You can't really get by without saying Sephiroth either. He went from a glorious hero to someone who was trying to destroy the plant, so that's pretty bad as well.

Of course Sephy-chan is the best villain ever! He's handsome, cool, I mean has gorgeous looks and he also strong. He has both beautiful long hair and insanity. Who can defeat him? Cloud can't defeat him completely and that makes him an eternal villain ever, right?
