So the screenwriter seems to want to imply and connect the fact that Kuniko's mind has been limited to only so much can be done in Duomo (her staring at the baby, at the hospital room, at the chocolate)
I guess to people who have been fed by the Internet this seems weird, especially anime nowadays. But worry not, the point is not Kuniko being ignorant and suicidal (lol the comments made me thought so), it's more trying to emphasize the 'you won't know until you see it'
And anymore strong posts like these (esp the shoutbox) and I'll get even more confused next time O.O
And so, this anime seems to gear towards more philosophy than action (e.g. KagaminSama was expecting someone to get shot)
But yeah cultural differences, I've seen people moving from place to place, class to class, and I myself have moved around quite a lot between countries, and seen animes depicting it. As tara said, it's basically culture shock.
Cultural shock is a concept that can't, or very hard to be understood by those who never felt it. Even now, as I've moved for years between countries, I still get cultural shocks due to generation shifts o.o
