ID Cards

Hello everyone!!

I just thought of a new system for the ID Cards. Because there are so many students in this group now, I'll get confused if I have to make cards for new students right after they sign up. I don't even take care of the signups anymore now, so it's hard for me.

So, this new system is, if you want to have an ID Card, you should just sign up here ^__^

Fill the form up (it's basically the same as the one you filled in your registration):

FOR STUDENTS

FOR TEACHERS / SUPPORTING STAFF

Notes, or rules or whatever you call it

1. NO MAKING UP CLASSES. If you're in class 2-C, put 2-C, and not 2-B or 1-C or anything. And as you know, YOU ONLY HAVE ONE MAJOR and the rest of the instruments you're learning here goes to Others.

2. Full names and nicknames MUST be the same as the ones you used during sign ups.

3. All pictures must be in JPG form or PNG form. PLEASE CHECK before you submit the pictures because if you give me GIF ones I won't be able to put it in the card.

Old students who already have their cards please sign up again if you had a class switch e.g. Melodia Class => Normal or the other way around
