well, madoka's wish was to destroy all witches, not make every unhappy thing go away. plus, there are some that speculate that the accident actually allowed sayaka to get closer to kamijou than anyone else, since she was the only one who visited him during his hospitalization.

However, think about it. She declared that she could stop sayaka from making the wish, meaning she wouldn't become a witch. I'm assuming that this was possible for her because it's technically stopping a witch from appearing. If kamijou hadn't hurt himself, sayaka would have no reason to contract, thus never becoming a magical girl and then a witch. It's the same thing, just on a larger scale.

Like how my friend told me that tenshi is the most powerful fictional character ever, because he affects a multiverse of multiverses. This means that at some point, the multiverses are connected making the multiverse of multiverses just one big multiverse. So he has the power to affect a large multiverse. He told me that haruka from noein could only affect her own multiverse. At some point, the multiverses are all connected, if you go back or forward far enough, so she essentially has the same power.

You're doing the same thing, just on a broader scale.
My point is, rather than thinking outside the box, just make the box a bit bigger.

Again, the wish was for Witches to stop being born. Meaning the world would stay as it is, pre Madokami, however replacing witch births with imminent magic girl deaths. If Kamijou's arm had nothing to do with witches directly, the wish wouldn't have affected him. Thus Sayaka would still have made that wish and gone into despair. Like how Mami and Kyouko are alive because their deaths were caused by witches.
I think Madoka's words were "In order to save you(from being a witch), this world will be left as it is(she sitll dies)." Or smthn.
She is powerful enough to change history, but far from being omnipotent, anything beyond Witches is not in her jurisdiction.
