It depends upon your ISP. If you have a major ISP, then as I mentioned above it most likely is already happening -just with higher bandwidth caps. If you have a more local ISP, then chances are that you do not have a bandwidth cap.

One of the problems that internet users have in the USA though, is that ISPs will frequently load their Terms of Service agreement with a claus which essentially says that they have the right to terminate or penalize you however they want if they ever find that you are "abusing" their network as determined by them. Fortunately ISPs in the USA have rarely used this claus except in cases where the user was engaging in extreme piracy (downloading hundreds of movies every month, etc...)

But as more and more people become heavy internet users, yes, this will become more and more prevalent here in the USA. Currently the heavy users are heavily subsidized by the light users who pay the full price for their "unlimited internet" but in fact are only costing the ISP a couple of pennies every month. As an ISP's light userbase decreases and their heavy userbase increases (or even remains stagnant), they will be forced to either increase prices to offset costs or to cap internet usage.
