Tetsuo: The Iron Man.
Some people won't be able to stomach it, and some may just find it off putting. Heck, I'm sure some may just laugh at it. But I always tend to put myself in the position of the characters on screen, so body horror can really freak me out. Anything by David Cronenberg is sure to get me to wince and be a little freaked out as well.
