i'd rather point out the anime studios than the anime itself,
anime by kyoani, ufotable, p.a. works, sunrise, production i.g., bones etc are usually top notch

In fairness, episode 30 of Naruto is genuinely impressive as the whole episode was helmed by Norio Matsumoto (veteran key animator), but on the whole it's got terrible animation.

Films: Most anime movies I've seen had impressive animation... not sure which is the best, but I agree that 5 Cm per Second is stunning, and The Sky Crawlers has stuck in my head since I saw it, Production IG have just gotten better and better with films over the years (except when they give you a WTF moment with that samurai thingy that was mostly a documentary), really fluid, well observed and timed character animation (and the designs were very elegant). On that note, Jin-Roh and A Letter to Momo were also exceptionally well animated. IMO, Ghost in the Shell is starting to look rather underwhelming by comparison (though there are still some very good bits of animation in it). Ghibli films always look good, Ponyo was particularly outstanding. Akira still holds up well in terms of fluidity and novelty, though the timing's a little off here and there (not out of sync so much as just too fast, thinking of that early sequence in the bar). A Tree of Palme has exceptional animation and design (director was the animation director on Akira, Takashi Nakamura).

TV series:

I'm surprised no-one's mentioned Birdy the Mighty Decode, that had very high quality animation for a TV series.
I'd also nominate Toradora, I was pretty impressed by that, though the quality dipped a bit in the second half.
Noein had a lot of really interesting animation in it.
Kemonozume and The Tatami Galaxy (and Kaiba is similarly beautiful from what I've seen).
Probably Anohana and Fractale, though I haven't seen all of either yet.
BONES series tend to be significantly above average to excellent animation-wise, best looking one so far is probably Heroman (not that I'm being all jingoistic or anything, I just was really wowed by the animation, and I like the show as a whole), Xam'd was also gorgeous, though IMO otherwise a mess and a retread of Eureka Seven.

I don't think that being on model is the mark of good animation, if they're just plain sloppy because they really can't draw that's one thing but actually going off model is considered to be a hallmark of good animation according to the 12 principles of animation. And certainly I like drawings to have character, it can be really beautiful when the hand of the animator is apparent in anime in a way it often isn't in western animation. I don't tune into anime to just see uniformly on model hot/cute moe girls ala Kyoani (though I do think their animation quality is very high/impressive, especially in their more recent shows.)

Also, from what I've seen Future Boy Conan has very good animation compared even to a lot of newer shows (though maybe the quality goes down as it continues).

I consider good animation when I can't tell the transition between frames. If characters move too much from one frame to another that is bad animation. Anything else I don't care it is good enough.

I think Horizon in the Middle of Nowhere has pretty amazing animation, and watching it in 1080p makes it even better!

It depends on whether you're asking about TV or movie animation. Movies will almost always have the best quality, and among the highest I would rate are stuff from Studio Ghibli, along with "The Girl who leapt through time" and "Summer Wars". TV wise, Fate/Zero is definitely up there, some of the stuff in K is also ridiculously good.

Also, although not anime, as far as animation goes I have to rate old Disney extremely high aswell, not to mention I sat through "Feivel Goes West" again the other day and could not BELIEVE that movie was made 21 years ago. I'm also not opposed to CGI, but in general I find it dates faster than 2d and is missing the warmth of the whole handcrafted look.

Most TV anime from the 2000-2008 range had good animation quality, at least the ones I watched.

I guess this is pretty subjective, but the most impressed I have ever been by animation was when I watched Akira. The realism of movement is incredible. I miss hand drawn animation...

Other than that Ghibli of course, and Makoto Shinkai films (particularly 5cm and Hoshi wo ou kodomo)

Cowboy Bebop and Samurai Champloo are pretty awesome too.

I enjoyed moribito's first fights scenes, they arent fluid per se, the character movements are. Its weird to explain it, overall it seems a bit choppy but the way the characters movements are drawn flow smoothly even though the overall framerate takes a hit

Code:Breaker, Fate/Zero!, Shinsekai Yori!, Guilty Crown, Toradora!, Tonari no Kaibutsu-kun!, Chunibyou Demo Koi Ga Shitai! and K !!!!! Naruto Shippuden has the crappiest animation as of late, they simply reuse it and there doesn't seem to be any detail in the characters, etc (zetsu's, water, etc).

fate/zero imma let you finish, but i think the monogatari series has the best animation quality of all time.
