Grave of the Fireflies.
5 Centimeters Per Second.

I can't even think about 5 Centimeters without getting choked up. Clannad has been in my queue for two years because I can't bring myself to watch it knowing it'll wreck me. One day I will, just gotta make sure my family won't be around when I do.

Saddest for me, in no particular order:

Blood+
Air (so sad, and that OP song!)
Angel Beats
Hell Girl (Just finished '3 vessels' set)
Certain episodes of Gunslinger Girl

and, believe it or not, Witchblade (one of the first anime I ever saw, sad ending)

Angel beats, clannad, guility crowm(could have been better ending),school days(gave everyone the WTF bomb) and kannon. I didn't sort it by order but they're all sad. Luckily I only got depressed after watching all of this!

Now and Then, Here and There is probably the saddest anime I've seen. Wolf's Rain and Samurai X: Trust and Betrayal had sad endings too.

I didn't find Grave of Fireflies all that moving; Seita comes off as too proud and stubborn for me to sympathize with.

Now and Then Here and There is a very sad and dark anime
Angel Beats! is another really sad one ( I have to admit I got teary watching this one at the end)... I'd like to watch angel beats again but I'm not up to it yet. Those are the two saddest ones I can think of right now

Oh yeah, Grave of the Fireflies. I think I got about a third of the way through, but couldn't finish it. It was too much

The saddest I was able to finish was Toward the Terra. I remember seeing a MAL fanclub for the series, which had a thread that asked people "How many times did you cry?"
