I guess it depends on what you consider realistic when you answer. Slice of life animess, would take the prize if you mean that no powers or fantasy and science-fiction is allowed. In that case, for me, Emma: A Victorian Romance, Sakamichi no Apollon, Uchuu Kyoudai, Hikaru no Go, Kare Kano... I have bad memory, I'm sure I know more. xD

Say I love you would be a close contender, but I've never seen love at 1st sight happen personally. I know it exists, how some of my family got together, but I have never witnessed it or experienced it.
Monster is another good contender, I just think the double ganger scene was a stretch into the realm of fantasy. Hyouka was the most realistic, personally speaking. |

I don't really pay attention much to realism. The reason I tend to enjoy anime more than most other mediums has a lot to do with the idea that animation isn't trying to be real... it's trying to be ideal. In fact, I feel like most TV shows, Movies, and Art in general is at it's best when expressing ideas as purely as possible. A lot of the most ridiculously unrealistic things that happen in anime (I think) seems like a byproduct of the creators taking certain ideas to their logical extremes, and if there's one thing I think most people can agree on, it's that anime can be fucking ridiculous. That said, I can still name a few that express a nice sense of reality:
