I don't think it's out yet. Every site I've checked has the release date slated for 2013, but no actual "date" (as in month, day, etc.). AnimeNewsNetwork has a page up for it:

It does list the vintage at 2013, but nothing else. Best info I can find is that it was delayed in late 2012 with a "future" release date sometime in 2013.

Kizumonogatari shouldn't be in Monogatari 2nd Season, the books announced for that are Neko- (White), Kabuki-, Hana-, Otori-, Oni-, and Koimonogatari. Unless that's incorrect information on their part (source).

It'd also be a bit of a mess to include chronologically.

I haven't read anything about Kizu- since it was pushed back last year, though.

Kizumonogatari is still TBA, no fixed date. At this point probably not until after some other stuff is done as Shaft is busy with Monogatari Second Season, Madoka Magika movies, etc. Since it will be a movie it will be a long wait for the Blurays anyways as Madoka Magika 1-2 movie Blurays are finally coming out next month and the movies have been out since last year.

Monogatari Second season is coming in July and includes Nekomonogatari Shiro (White Cat Story), Koimonogatari (Love Story), Otorimonogatari (Decoy Story), Kabukimonogatari (Leaning Story), Hanamonogatari (Flower Story) and Onimonogatari (Ogre Story).

No release date for North American Nekomonogatari Kuro yet. Some have speculated that they might include it in a boxed set with Monogatari Second Season since they tend to wait and just make boxed sets for North American releases. Just a guess though. I have the import discs.

No, not for any of the Monogatari TV series import discs I'm afraid.

The Madoka Magika movies do have subs however so there is hope for Kizumonogatari movie, when it comes out on disk. The TV series discs don't have subs.

Ghost in the Shell: Arise is supposed to have English subs in its Japanese release as well.

So things are at least trending in that direction.

Import discs of LoveLive! School Idol Project and Little Busters! have English subs. Gargantia will as well. Gurren Lagann has subs and dubs.

I need to try and learn some Japanese, I read a translation of Kizumonogatari but by God was it painfully done.

Although with all the wordplay that is involved in those books I can hardly blame the translator since I think they probably did the best they could.

OMG, you really need to read it in Japanese! Nisio's writing in Japanese is so beautiful! It is so well done, the anime gives you a little taste when they flash the characters by. He writes so well, I don't think it can be appreciated in a translation.
