This is one of the biggest pet peeves I have about watching anime. You watch an entire series, only to find out that the manga is either ongoing or the show was given its own ending that in retrospect has no relation to the plot whatsoever.

That said, I'd like to establish an up to date list of anime that actually follow their source material.
There are a few rules I ask if you wish to contribute however:

Any anime that follows the source material for the most part is allowed
Ongoing anime that are currently following their source material are also allowed
If an anime has either been cancelled or its season has ended, yet the show follows the source to said end point, that is also alright.

Of course discussion of this matter is also more than welcome.
WARNING SPOILERS MAY BE IMMINENT!!!

Now let's start this list off:

One Piece: (several filler arcs) Ongoing
Naruto: (1 year + worth of filler between both halves of the series) Ongoing
Attack on Titan: Ongoing
Bleach: (several filler arcs) Cancelled Before Final Arc.
Full Metal Alchemist: Brotherhood: Completed
Accel World: (season one ended with many unanswered questions) Possibly Ongoing

Accel World season 2 is in the fall.

im inclined to say YAY

Sakurasou no Petto na Kanojo: Finished 6/10 novels, volume 10 comes out next month.
My Sister is Among them / Nakaimo: Ongoing, not finished, did about 9/10 novels.
So I can't play H / Dakara boku wa H ga dekinai: Ongoing, not finished, novel 14 just came out.
Campione!: ongoing, anime well behind, novel 14 just came out.
Problem Children are coming from another world aren't they / Mondaiji: Anime only finished about first 2 novels, 7 are done, 8 comes next month with OVA.

I'm confused,

Are these anime you're saying are completed or light novels?

All of these are anime which have NOT FINISHED their source material. There is more material after the anime finished.

In two of the cases the novels themselves are done publishing but the anime does not cover all the published material for any of them.

Death Note followed the manga almost to a tee.

D.Gray-man followed the manga.
Since the manga is ongoing they gave the anime an open ending
instead of just making one up, which I approve of whole heartily!

Durarara!!, though now that I think about it
I'm not really sure what came out first, the manga, anime or light novels.
I think I heard somewhere that they all came out at the same time but I don't know.
I do know that the manga and the anime are almost identical up to the point when the anime ends.

Karneval is currently airing and it's following the manga for the most part.
There are a few small things that have been left out but nothing major as far as I can recall.

I like how you colored the anime titles the corresponding colors, but did Death Note get cancelled or is that just for creepy effect?

Gintama followed the manga, though the manga's still ongoing. There's going to be a movie released next month, but I think after that they're supposed to continue with the anime.

Katekyo Hitman Reborn followed the manga, (except for the absence of Longchamp) though the anime stopped directly after the future arc. There have been rumors that since the manga recently finished, they're planning on continuing the anime, though I'm not sure.

Kokoro Connect ended the same as book 4, no BS/anime only ending. So... it is possible though unlikely that they could do another 1-2 seasons given that there are 6 more books written and the books get longer with each one as often happens with these series so there is really 2 cours worth if they wanted to do it or 1 cour if they wanted to be selective. Especially considering that season one was 17 episodes so they need about 4 per book = 24 left.

The Higurashi No Naku Koro Ni series is also quite faithful to both the manga and the visual novel
