A lot of anime are horribly adapted because of incomplete manga or the animation studio wanting a completely different feel. (EX: Chrono Crusade)

Even though the anime doesn't follow I still enjoy them and will watch them but I'll also read the manga after to see what is different. Kobato, Chrono Crusade, Dance In the Vampire Bund, Dusk Maiden of Amnesia, To Love-Ru, To Love-Ru Darkness, Elfen Lied, HighSchool of the Dead, and Rosario+Vampire. I thought Death Note, Fairy Tail, Fullmetal Alchemist, Inuyasha, Jormangund, and K-On! followed the manga pretty well. Reading Future Diary right now to compare them.

I find both Naruto and Bleach are horrible adaptations of the manga. I'd love to have a truly well-adapted (ala FullMetal Alchemist Brotherhood) anime that consists of mostly only manga material, but for some reason they have horribly inconsistent teams and thus levels of accuracy (not to mention fillers). There are some wonderfully adapted episodes, but they're few and far between. Those two manga-to-anime adaptations have been my biggest headache.

Other than that... Attack on Titan because S2 isn't out yet...and probably won't be for a while...

Also, not a manga, but a visual novel: Togainu no Chi.
... Where to begin with this one?
The awful animation (you know you've screwed up when the very first DVD is delayed), the way they tried to cram aspects of every route into twelve episodes and the way it was so rushed as a result, the generally poor writing, among other things.
There were a couple of minor plot holes, too.

I will give it credit for making me laugh with this scene, though.
Mostly because Rin reminds me of a friend so much, there.

Agreed. The anime for Togainu no chi was terrible. It was really boring and I felt like it was going no where most of the time. The animation definitely could've improved, especially since there were scenes that looked really half-ass drawn. I was so disappointed with the ending and just the whole anime. I ultimately felt like it was a big waste of time. Eventhough I feel like the manga ia still kinda boring it's definitely a lot better than the anime.

My vote is for Love Hina. Despite the bonus of having Hocchan voice Naru, the anime waters down most of the things I enjoyed about the manga (e.g. it never goes into just how badly Keitaro screws up, or how much of a loser he comes off being most of the time). It also adds things that make next to no sense (e.g. that Kentaro guy and his spaceship) while leaving out stuff from the manga that it could easily have used.

I also vote for Video Girl Ai, not because the anime is bad, exactly, since it's actually pretty good, but it covers so little of an excellent manga – my absolute favorite, actually – that it's very disappointing for those of us who were hoping for more.

My most hated anime adaption is Blue Exorcist, for obvious, infuriating reasons.

I don't mind the Magi adaption, but I really, really wish they hadn't done that original ending for the first season.

Oh, A-1 Pictures, why? When you've done so much better with Space Brothers and Silver Spoon.

Definitely Beelzebub. Right from the start it was too light hearted and the humor felt kind of different and babyish for some reason. The animation was terrible. They made the anime way too light hearted since they left out all the tension, blood and tojou's incredibly badass entrance that was supposed to be a complete brutal beatdown of a gang with a motorcycle smashed into a concrete wall, but in the anime it was just a puff of dust with tojou walking off. I mean COME ON!! How lazy can they get? I still can't let that one go.

I almost got the feeling that the anime rubbed off on the mangaka because right after the incredibly badass jabberwock arc the manga turned all light-hearted, zero tension, with the same kind of babyish humor the anime had. And almost none of the arcs after it had to do with the main story, almost like fillers. I'm not sure if the anime made me see it that way or the manga had really changed.

Oh, yeah...I forgot one of the most obvious ones:

ZETMAN

The manga's not even over yet and the show ended things in one of the worst and weirdest ways.

MIRACLE GIRLS. I read the first volume and I was like shit, this is pretty good, but I couldn't find number 2 so I tried to watch the anime. Biggest mindfuck I've ever experienced. I wasn't even sure if I was watching the right anime. o.O

I LOVED the Blue Exorcist manga and I did find the anime wasn't as good, but I didn't find it bad. Mind explaining why you did?

I didn't like the anime original ending, which verred waaaay off the manga's plot, and they added a bit more fluff/filler, which I didn't appreciate. But your mileage may vary.

Maybe I missed it but Needless's end changed a few character's around due to the incomplete manga at the time.

Oh yeah, I get that. I didn't like how they went so off track from the manga either, but I figured it could have been worse so I just shrugged and thought it was good enough.

Love Hina's anime was a god damned mistake. Cute manga that was absolutely destroyed by picture quality and horrible voice acting (both Japanese and English versions were god awful). Not to mention the fact that it totally diverged from manga's plot.
