Both seasons of Freezing the manga is one of my favourites and I feel like the terrible adaptations is one of the reasons I can't own the manga in English.

Definitely Mahou Sensei Negima

There's more than one adaptation of it. They actually had some special high quality episodes released to animate the entire last story arcs, so you shouldn't call it the worst.

As far as the worst adaptation I've seen, I haven't seen many bad ones, but Love Hina's would probably count as a bad one, because I really liked the manga.

And on second thought, Kimi no Iru Machi definitely had a bad adaptation.

Rosario X Vampire, they turned pretty damn good story in to a pathetic boob fest, specially in s 2.

Kurogane no Linebarrels and the anime didn't do any justice for it's source material.

Really I mean REALLY compare the final filler arc end to blue exorcist and the arc that was going on in the manga at the time. If you can't tell me the fire king/ that one exorcist becoming a demon arc was the worst arc so far in the manga then I have nothing to say to you. Overall the anime was nicely done and yes it did have a filler arc ending to it. better a filler arc ending than a cliffhanger to a lead on to a second that WE WOULD HAVE NEVER GOTTEN. At least animes with original endings don't happen as often as they did in the early 2000's
If y'all thought blue exorcist's ending was bad try watching a unfinished no second season anime in the year 2004.... Those were so bad.

I have never seen "Suzuka" mentioned before. Was it that awful? I agree with the 2nd season of "Freezing".

Out of many that I've seen I was the saddest about Mysterious Girlfriend X/Nazo no Kanojo .The animation was pretty good but the adaptation of the storyline was horrible. They were all over the place They should've stuck to the manga's storyline.

Sorry gotta disagree...

The anime was a masterpiece. If it wasnt up to the manga that was its only failure.

Another vote for Kimi no Iru Machi. The anime was horrible compare to the manga, makes me wonder wtf they were thinking attempting to squeeze in 140ish chapters into 12 episodes. Brynhildr in the Darkness is another one that comes off the top of my head as of now.

Elfen Lied, although any kind of second attempt probably won`t happen. Brynhildr in the Darkness was recently screwed up pretty bad as well. I like Okamoto as a Mangaka, but really wish they could get some of his series actually adapted right. I don`t foresee that happening anytime soon unfortunately.

When I first watched it, I though Kotoura-san was a good anime. I changed my mind after reading the manga. I can't help but feel the anime cheapened the story quite a bit.
