I'm giving it to Toradora.
I've seen many others, but no one else have the mixing characters, situations and story toradora has.
Besides, the story is also similar to the one I had with the first girl I liked

My personal list top 7:
7. Otome Youkai Zakuro - One of my all time favorites, Kai and Zakuro's love is fantastic. If it only had more and if they would have actually shown the kiss, it would rank higher...

6. Spice and Wolf - This would be #1 if they would ever finish the story, but the part they did finish is one of the finest, most realistic, and "grown up" relationship stories ever told in anime, despite the fact that one of the two is a who-knows-how-old wise wolf/harvest goddess. This isn't a magical girlfriend story, it is a story about acquaintances travelling together for convenience whose relationship grows from that to business partners, to friends, to lovers.

5. Gosick - This story says it is a mystery, but the real story is the relationship. The way that Victorique goes from a girl incapable of even understanding the concept of love to

4. Ah! My Goddess - Another that would move to first if they ever finished the anime, if the manga ever finishes, that is... Keiichi and Belldandy share the purest of love.

3. Toradora! - Ryuuji and Taiga's love is a thing of beauty to watch unfold. It is sweet, but with a bit of salt added in for extra spice. And the kiss scene may be the finest ever put to pixels, (Though the #1 on my list gives it a run for its money...)

2. True Tears - This little gem holds the honor of being one of the only anime love stories that left me wondering which girl the protagonist would choose right up until the moment he spoke the words. I really, honestly didn't know. It was impressive. The plot devices in the middle were plain as day and as predictable as the sunrise, but the contents of the young man's heart was a mystery to me, and to the girls involved. (I think they both thought he would choose the other, frankly... or that we were supposed to think that...)

1. Ano Natsu de Matteru - This one gets the nod over all the others that I have seen because of the complex relationships, the earnestness of the characters feelings, and the emotional impact of the relationships, both positive and negative. In particular, episode 9 is fantastic. It takes you to both extremes
From the depths of despair to the pinnacle of mutual love confirmed. All in the space of a few brief minutes, and sealed with a kiss...

But what makes it even better is that the story continues on after the confession, to the "new couple" period, when they just can't seem to separate themselves. The cuddling is the icing on the cake!

Honorable Mention:
Angel Beats - If they had developed the story more and not rushed the ending, this would be right up there.
Full Metal Panic - Another show that deserves to be animated to completion, FMP would join the list near the top if they ever animated the final novel: Always Stand by Me.
Blue Drop - If this had a bit more than hand-holding and didn't have such a gut-wrenchingly painful ending, it would be right up there. (I like Yuri romance... Hey, I have relatives that are lesbians and I think their love is beautiful! I would include a yaoi story if they had one that I didn't find abusive at the core....)
Kimi no Iru Machi - This anime, while paling in comparison to the manga, is still a fine love story. But I can't put it up in the top seven because a) it isn't done yet and b) I have a strong feeling that my love of the manga is influencing my interpretation of the anime... But I love it!
Mysterious Girlfriend X - It may be weird and some people may think it is too gross, but few anime deal exclusively with what happens after the confession. Urabe is one of the absolute best characters ever and the story is soooo sweet.

And, the story that I believe would truly top them all, if only the anime told the entire story from the manga:
Sasameki Koto - This story, in the anime, is sweet and pure and almost gets there, but, alas, it fails to make it to the finish line. After being massively disappointed in the ending of the anime, I sought out the manga and OH MY, to quote George Takei. The story in the manga is practically the perfect love story. Unrequited love, to mutual love that both think is unrequited, finally making it to the finish line, then moving beyond it. Not only does this manga tell a beautiful love story and have a lot of great comedy, it also strikes a blow for equality again and again.

I'd have to say Eureka Seven. It starts out as a simple teenage crush, but becomes so much more as the characters mature over the course of the series. I really like how their developing relationship is integrated into the overall plot, right down to the final episode.

Not only is the Renton/Eureka relationship great, but so are the secondary relationships (Dominic/Anemone, Holland/Talho, Norb/Sakuya, Charles/Ray).

Eureka 7 was a great love story, I will give you that. And as all the others have said, Toradora was quite good too, and this is coming me, someone who wants to strangle all Kugimiya tsunderes (I can't even listen to her voice anymore without feeling the urge to hit someone).

While Eureka 7 was great though, I must warn people who liked it and haven't seen the sequel, Eureka AO, that unlike 7, AO is not a love story. It may fool you in the first few episodes with that childhood friend chick, but really none of the relationships in the show go anywhere. AO is primarily about politics, and I think possibly eco-terrorism. In the end I really couldn't tell what the show was about, there were too many plots going on at the same time, none of which involved romance. So if anyone out there wants to watch AO for a continuation of Eureka 7, do not, it does a very bad job at that, it is a whole different monster on its own.

Spice and Wolf is finished! In the 17 volumes of Japanese novels anyways and the English ones are coming along! Needs a few more seasons of anime however... definitely one of the best ever, I need to retake this picture as I didn't have volume 4 at the time I took this.

Ahh My Goddess chapter 300 just came out last month... the manga love story is still going 25 years later!

Mysterious Girlfriend X is still going strong too, same magazine as Ahh My Goddess but only 80 chapters so far LOL

Other favorites of mine, some already listed by others:

Kokoro Connect: The anime only went 4 books in, 7 books left! Incredible love stories! Inaba and Taichi OTP but there are other great relationships too.

Pet girl of Sakurasou - again, the anime just didn't go far enough. Wonderful love stories, the triangle of Sorata, Mashiro and Nanami, Jin and Misaki, Rita and Dragon. Novels 7-10.5 need to be animated! So wonderful what happens by the end of 10, I posted a summary. It goes beyond the confessions, to dating and more.

ef - tale of memories: amazing, Amazing, AMAZING! Some of the most amazing scenes ever in anime. Marathoning the whole series can give you a heart attack when you get caught up in it.

SankaRea - again sadly the anime doesn't get far enough so you have to continue it in the manga, amazing eternal zombie love story triangle with the cousin Wanko.

Dusk Maiden of Amnesia - wonderful, Wonderful, WONDERFUL ghost love story and triangle with Teiichi, Yuuko and Kirie or even a quadrangle with poor Momoe only having faint hope.

Say I Love You - the wonderful love story of Mei and Yamato which, common theme, is not finished in the anime (it goes anime only in 13-OVA). The manga continues far further, further than most shoujo romances dare to go, and then beyond that!

My Little Monster - great story of Mitty and Haru, again the anime doesn't finish it but the manga does... and then the side story of Sasayan and Natsume begins!

Shinsekai Yori, this is the most complicated love polygon you will ever find in an anime! Yuri, BL, you name it, but very tragic.

Unrequited love stories 5cm/second, Garden of Words, Places Promised in our Early Days, Voices from the Stars

People are going to look at me funny but one of the best and my favorite love stories in manga and anime (just one 1-hour OVA so far) is in fact Nozoki Ana. In the end, after 13 volumes of manga it is one of the best mature, very mature seinen love stories ever told, explicitly, very explicitly but in the end it is an incredible love story which you'd never guess from the first scene. If you like your love stories graphic, this is for you. I am glad I didn't read this one monthly because I honestly, truly might have died if I'd had to wait a month between chapters for the last few, especially the last one. Even having it in my hands I had to flip quickly to the end, I just had to know...

Another one on the mature side is So I Can't Play H... You may see it as just an ecchi show/novel series but it is in fact a great mature love story which just ended after 11 volumes. Wonderful stuff if you like your love story with a side of H.

And yet another one, Nakaimo, My Sister is among Them. Looks like a siscon harem anime/novel only but in the end it is a love story too.

Another mature one is BTOOOM! Incredible love story of Ryota and Himiko that ends at a wonderful point in the anime though the manga continues. I know, it is about a survival game but their love story from meeting online before going to the island and realizing they are who they are in real life, just incredible.

In the end IMO all good stories are also love stories.

I'm sure I'm still forgetting some...
