Fooly cooly immediately comes to mind.

As do most Girls Love anime. For example...

Candy Boy - It's not about boys or candy, it's about incestbians.
Strawberry Panic - Doesn't relate to strawberry's at all and it's more about melodrama than panic.

I didn't find it that bad or confusing, because it reminds me of the Tales series of games. a lot of the games call the special skills as "artes" (pronounced arts). also, the game is essentially about the art of swordsmanship. I'm not sure if it was explained really well in the anime, but the world of SAO required at least somewhat of an understanding of how to handle a sword in order to progress.

The strawberry comes from the shape of the school dorm on Astraea Hill , they mention in the series that it's shaped like a strawberry and the panic comes from all the melodramatic relationship chaos that goes on. Not an official explanation just my 2 cents.

true, seeing as how that aspect of it isn't nearly as important as it was in the novels.

I always thought it was shaped like a vagina...Freudian.

When I hear panic I think natural disaster, people freaking out, chaos. Not angsty teens being angsty because senpai hasn't noticed me yet this year. Such a good/bad anime.

Another fun one: Getter Robo. Never did figure out where Nagai got that, so I just figured it was because it sounded like 'get a robo'. Because how else are you going to solve your giant monster problems?

And ChäoS;HEAd (thank you, copy-paste function) is fun. Dude certainly has chaos in his head, so it's not like the title's untruthful. It's just really difficult to type accurately.

I'd say Mangirl! could be a misleading/questionable title. The name actually comes from Manga + Girl, since it's a series about a group of girls who run an official manga magazine.

I actually overlooked that series initially because of the title. Without bothering to read up on it I assumed it was some ecchi comedy series involving butlers/maids. I'm ashamed.
