Oh wow, I adore reading fiction and have never had this problem so I would love for you to find an answer.
Maybe read a book that has (a decent) film adaption. Maybe that will help you connect the words on the page with images in your head.
As far as distracting thoughts are concerned; compartmentalise your attention (took me a while to get the hang of that one actually).
Think, 'Now I will focus on this and deal with this next' it'll stop you think everything at once because you've allotted time for it later.

Try making a timeline of events in the story as you discover the different plots/twists and string them all together. This is what I do when I cannot focus.

If there is a part i'm lost about whether or not a sentence, a page or a whole chapter I usually highlight where I get lost a dissect all the information. Another thing is to close your eyes for about 2 to 4 minutes I summarize that chapter that I read if I didn't get the chapter I usually just write down what that chapter in four sentences if it's more than four sentences I skim the chapter again to see if I can write the summary If not I read the chapter again and the second time I usually get what's going on.

Oooh I have the same problem, along with ADHD... or ADD can't remember which. I always read series with one particular artist's music playing. When my mind wanders it usually ends up on the music for awhile before than focusing back on the book. I remember reading all of The Wheel of Time series while listening to NoGod, certainly didn't fit the story, but I found it easier to focus for that one hell of a long series.

Light novels are a boon of gods to my problems as they don't go too deep for me to lose interest. And Kazuma never fails to write a good LN that interests me both for entertainment as well as contemplation.

Also Visual Novels, they keep the text, and awesomeness of a book and mix it with the best of anime for great masterpieces.

watch something and close your eyes and imagine what you see. or you could imagine something you havent seen just like when you draw. xD IDK~

It depends. people generally think to hard about simple things(good and bad) causing themselves stress. generally when that happens to me i just come here to CR for a few hours and usually feel better. losing yourself in another story is always good medicine imo.

Well I have no experience with this issue... only because I never read.. xD Unless when I'm on the internet, or for some homework, but I have never read a book out of joy, don't know why, probably because I have the same problem, but I just never noticed.

But what I would say, find a relaxing spot, maybe lie down, sit in a very comfortable chair, something that would make you just relax all your body, then take out your book and try reading.
I think where the problem lies, has mostly to do with, as someone said above, ADHD, which is probably why you can't focus as well (which is the basic definition of ADHD, you can't focus on one thing at a time)
BUT! I might have a solution for you, or those who have it, but this requires you to be able to turn the page and hold the book with one hand :P

What you do is.. Take your other hand give it a ball, pen or something to keep it busy, and just let the ADHD take over from that hand, let it do its magic, this might not really work for everyone, but those who always tend to want to move their legs or just hit the table with a pen or something like this, this would most likely help you concentrate while reading a book, or at least a bit better :)

Hope this helps, but once again I will just remind you, I'm no expert, nor do I have experience. This is only things I've heard from teachers and some things that I've read online, don't think all of this is true, but it wouldn't help to try if you're out of options :P

I don't get stressed too much since nothing is really happening right now for me. Yet I do get nervous about things that wouldn't cause one to even get a little nervous about. My thoughts do stop me from concentrating on the story because I go from thinking positive to plain negative on ANYTHING. I guess the nervousness can count as stress though.

My POINT , is that : whats a book without different elements happening at once? Don't think too hard about it or you miss EVERYTHING that's going on or you'll NEVER understand the point of the book. Think of it as a movie where as there are different things happening at once or at different times.

so my POINT is: ENJOY YOUR BOOK for what it is!

I do the exact same thing, OP. Sometimes, I cannot focus on one plot, and my mind just wanders. I let it wander. Usually I only read a couple pages at a time, then sit and think about those pages or whatever else, then read a couple more. I have issues reading through a book really fast. I need time. Soak it in.

I've also tried to plot out my own stories, but whenever I get really involved in one, I'll start getting bored or distracted and start on another. My computer is full of notes on at least 10 different ideas. It's ridiculous.
