Suoh Mikoto from K Project:

While he does have the power of flame, if he uses it to much or loses control an explosion will occur at the same time as this Sword of Damocles lowering and eventually falling and killing him.

Tsukune Aono from Rosario and Vampire. His vampire powers are turning him into a zombie.
Koyomi Araragi from Monogatari. His ability to survive his brushes with apparitions also draws them to him.

gintoki from gintama.

Gift:
His greatest strength seems to be his immense stamina and endurance, to the point where he has been called immortal by other characters. He is protective of his friends and willing to sacrifice himself for them. He can also use final getsuga tensho.

Curse:
He is a dick, a six timing bastard, a gambler ,alcoholic, childish, lazy and has curly perm hair.

to use your powers you need to do something that you hate. hei is killing, but the irony is his ability to kill.

also when you think of equivalent exchange theres non other than alchemy.
learning the secrets to alchemy and you'll lose something such as al with his body, ed with his arm and leg, mustang with his sight, and the teacher with her insides.

also i think you should mark the top stating that everything in here is a spoiler because of its given nature.

That opening scene in Darker than Black was hard for me to watch, something about breaking bones always sends chills up my spine. That's definitely a hell of a price to pay to use your power. One thing to note is that I don't think all of the prices were things the contractors hated, or were even especially unpleasant. Most of them were just very arbitrary and inconvenient.

Aside from that, obviously being a Valvrave pilot kind of sucks.

you my friend have started a war. jk

anywho, arbitary things are something the things that lead many people to kill another or to simply end their lives. and its something they face everyday. i also forgot the fact that they lose their emotion, some remember what it is and other just lose there humanity. so on many levels those arbitary things are hell.

also i haven't seen the second season, but plan to. only the main character seemed to lose his humanity. the others just seemed to gain abilities and responsibility.

Any King's Candidate from Magi. You get awesome superpowers but the more you use them, the more magoi (basically life energy) you drain. Use it too much, and you die. There's also the whole issue of being affected by black rukh, which turns you evil.

Also, if you choose to use dark magic, it could very well cost you your life. You can obtain great power but first you have to pretty much kill yourself.

This poor dude decided to sacrifice himself so that he could turn into this and annihilate people:

Light Yagami from Death Note. Technically not really a 'power' because he is a sociopath and his lack of sympathy and empathy towards the people he brutally murders is just the average thing for a sociopath, but I considering the power to murder people so easily a blessing and a curse. (For him, technically, not for anyone else) Because he can easily pave his way for a new world, but to do such a thing he must become evil himself, and obviously there is nothing he hates more than evil. He'll never admit he's evil though, and has deluded himself into thinking otherwise.

And the reason I'm describing this as a type of power is because people do not normally possess the nerve to murder a person, let alone murder thousands of people without so much as batting an eyelash. Being a sociopath to Light's level is not a good 'power', but sort of one in the end.
