If Crunchyroll gets the initial rights now for a few countries it can pave the way to get additional rights for other countries in the future. If the anime's production team, creator, etc, sees that Crunchyroll is pulling in a lot of episode views then they will be much more willing to make a deal with them for the rights once the current contract expires. If they don't take the show at all for any countries then it is a lot less likely that they will ever be able to get the rights for it. For all you know the UK could still be under contract for another month and would be added to the list of countries at that time. You can't just go around breaking contracts unless you want to open yourself up to lawsuits.

The initial list of simulcast countries is not final by any means. I have seen them add countries before and I am positive it will happen again at some point. If they take the opportunity to start streaming One Piece now, it is much more likely that they will be able to add more countries once the current contracts expire.

Normally when it says "Latin American and the Caribbean", or "Central and South America and the Caribbean".

Yes, certainly ~ whoever holds the rights handed over money in some way to gain those rights, and they recoup that money, if they do, with the revenues they earn from the royalties.

The details can range from simple to complex, because its determined by how the contract terms are written, and how strong the relationship is between the companies.
