Serial Experiments Lain is the top mindf%$k anime I've ever seen. Have to give that the number one spot. Don't be turned off by the fact that is screws with your head, it still is a great show.

Other notable anime:

Kara no Kyoukai - The Garden of Sinners: If you haven't seen it you should, amazing 8 movie collection

Higurashi/Kai - You have no idea what is happening in the first season until the last few minutes of the last episode in season 1, then it hits you like a brick wall. Almost a necessity to watch season 2 after that.

End of Evangelion - The end of the world never was so beautiful!

Monster and Psycho-Pass: Probably best antagonists in anime I've seen thus far

I'm watching Serial Experiments Lain right now and I feel like I know what's going on but really don't. Not to mention how some of the things that happened makes no sense to me yet. It's fucking with me right now but I love it. Great storytelling if you ask me. I'm only 7 episodes in though.

For me, it was the movie Jin Roh--but probably because of my background in both police and military. Still, scary stuff!
Interlude was pretty messed up, and I still haven't figured out the ending of Big O! (Although that last one has me more irritated than anything. . .)

I would say the darkest or most disturbing of those would be Higurashi and Higurashi Kai, then Monster. The others are great, but are more mind games than dark. Id say Kara no Kyoukai - The Garden of Sinners is probably the best example of a middle ground; overall dark theme and some episodes (movies actually) you might have to watch a second time as they go in and out of chronological order and might mess with you a bit.

Texhnolyze. It's by the same creator of serial experiments lain. The show starts off a little slow, but it is really good and probably the one darkest series i'v seen.

I've seen several..Gantz, for one. After seeing that..well it's very unsettling. Another one is Gilgamesh. There's just something about this anime..I can't articulate. Then there is Shigurui: Death Frenzy. It's gory, it's horrifying and based on historical fact, truly..mind messing caliber.

Wow. I never thought I'd see the words "I have never heard of Akira" on an anime site. I'm not saying it's the greatest anime ever or anything, but back in the day, it was hard for me or my friends to say the word "Anime" without thinking of Akira. Maybe I'm just showing my age.
