Originally S was a qualification of the highest rank safety variance in the toyota production system (an S-ranked defect). This was well before video games but this ranking system spread in japan to include other manufacturing and eventually flipped and used as "difficulty level". SS, SSS, and others are purely fictional to the game world rankings to create rarity and difficulty. Unused in video games there is also an N ranked defect but this came about much later in the auto industry and is unfamiliar to most not associated with it.

I think it is just a way to differentiate between a large gap in skill. If you are a C-rank whatever (ninja, mage, etc) logically B is greater and A is even higher so "S" is used to show that (ninjas, mages, etc) ranked that are of even greater skill than an A-rank because there is no letter before A and S is in its own category.

On another note I've never really thought about it before you asked lol, suspension of disbelief

She only applied to a synthetic rank. In firepower, she's the strongest of them all. In terms of combat skill.... she's probably E or something.
