The ending to Akira.
Those two moments in Elfen Lied, when the children murdered that puppy, along with the murder of both the boy's younger sister and father.
The ending to Code Geass R1 and R2(mainly R2) then Euphemia's death scene.

Just my opinion.

That one episode/ episodes with the crazy artist girl in Psycho-pass. Dear god.

And the higurashi fingernail scene. I was reading the manga and I was like shaking and closing my book every five seconds. And then my friend is like "what's wrong?" And i'm just like "YOU DON'T WANT TO KNOW. PROTECT YOUR FINGERNAILS WITH YOUR LIFE."

Because it was one of the first anime I saw when the puppy was killed in Elfen Lied.

I have many but the most traumatic one that made me want to vomit was in Rainbow: Nisha Rokubō no Shichinin...I wasn't prepared for those scenes

When Eren got eaten

I actually stopped here. The next episode was a flashback episode and Eren getting eaten felt like closure.
Turns out the anime's plot isn't complete after one season and a lot of people's complaints have been about the plot armor that some characters have, so I'm honestly glad that I stopped there.

"AoT: A show that involves titans attacking and ends with Eren eaten." I'm thoroughly satisfied with this.

There was a part in episode 5 of Elfen Lied. When I saw it, I was honestly just... I don't even know. I had to quit watching the whole show at that point. I just couldn't deal with it anymore. I mean, I guess i shouldn't have been shocked by this show - but there's only so much I can take or expect.

Similarly, episode 11 of Fractale as well.

EDIT -

Quasi-serious note

I think these shows really did sort of "traumatize" me in a very mild sense. I know this kind of stuff happens in real life (which is just wicked), but I don't ever want to see it. Even in a fictional setting. Those shows have almost made me hypersensitive and worried because I know that some of the more serious shows will go there... Honestly, they may have spurred me away from serious shows and indirectly solidified my preference for lighthearted slice of life.

Definitely the dog scene in Elfen Lied. I can handle any sort of violence, I was chuckling to myself during most of the carnage scenes because it's so over the top. But...
