Second this. All of the Deep Love series are very good. Although I try not to think of them too much as they are some of the most depressing things you will ever read. This is one series I always hope they don't make any more of, and not because I think that they don't deserve more or that the story is finished the way it is. I don't want them to make more because the author is a sadist who gives us rays of hope only to crush dreams. Every continuation seems to be about crushing the little light that the last one had.

Koe no Katachi. Pulls at your heart strings, has strong characters, a good pace to the plot, and isn't too far into the story so you can catch up pretty easily. Also CR has it now so it's easy to access

edit: got carried away with the one and decided to put all my favorites... still these all deserve to be 'the one' you always recommend to a friend, just depends on the friend i guess
Dogs: Bullets and Carnage, for the art style and ambiance alone (and every fight scene is so badass and perfectly choreographed).

if you like Kare Kano's brand of realism try these:
Bokura ga ita, first shoujo I read that had such psychologically realistic and moving characters.
Oboreru Knife, amazing artwork and memorable, flawed characters.
Koe no Katachi, for all the reasons recommended by other users. the original oneshot already has everything that makes the series great.

A Cruel God Reigns by Hagio Moto, omgod just... rd it.
anything by Ai Yazawa, Nana is amazing but Gokinjo Monogatari is so much fun and so is Paradise Kiss.
also Nakamura Asumiko's work (like J no Subete) is so twisted and haunting, the artwork is so gorgeous, her more comedic stuff (Doukyusei, Sora to Hara) is also very charming, don't let the BL label put you off.

most of the above (except for dogs) I would put in the hurts-so-good category, really emotionally involving

Definitely Kingdom. It's got the uplifting essence of a shonen jump manga which balances out the violence/gore enough so as to not become as dark as Berserk. Utter perfection. It's got some high ratings on most manga sites (#23 on the one I frequent), but I feel it really doesn't have the exposure it deserves. It's really good!

I'd second Koe no Katchi, I'd also recommend Ore ga Doutei o Sutetara Shinu Ken ni Tsuite. Both mangas share a "putting things right" theme.
