I already carry a fork and spoon in my lunch box thing for work, since I always have to bring in my lunch for work because I don't drive and there aren't any decent fast food places around close enough for me to walk to and back while on my lunch. Wouldn't be too hard to just add a knife in there and then I'd be all set. Sounds like a perfect idea to me. Let's start a revolution!

Solution for cases 1 and 2: Bring your own soap and/or hand sanitizer and bottle of water or own personal water bottle wherever you go. That way you can wash off the utensils and rinse them off wherever you are. You can wipe them on your clothes to dry them, or bring along a cloth or handkerchief to dry them off with.

Assuming that the menus don't entirely consist of finger-food; if restaurants with prices exceeding $15 cannot be bothered with supplying even plastic utensils, much less, can't wash dishes or keep up with them properly, I don't consider them worth going to. Actually, I wouldn't want to knowingly step foot into even fast-food joints that harbor workers who can't decently clean. I dine at restaurants or obtain fast food 1-5 times a year, and certainly won't lose any sleep over entirely cutting them out of my life.
