I have seen most everything mentioned here but nothing even comes close to the final 2 episodes of Anohana IMO.

One moment. *rifles through anime list*
The most depressing animes I have ever seen:
Bolded are the ones I recommend
Amnesia - The process of Heroine trying to figure out what alternate world is real made it hard for me to watch. The ending is what made it especially sad for me.
Another - Needless violence and killing was what did it for me and the ending was the cherry on the cake.
Anohana: The Flower We Saw That Day
- Slice of life shows always hits me deep. I've never lost a childhood friend before, but if I were to, I'd like to imagine this is how I'd honor them.
Boogiepop Phantom - Maybe it was the dark animation, but I had a sense of dread as I was watching this series.
Usagi Drop - It's never easy losing someone you were close to, coming from someone who hasn't lost anyone yet. Touching story about learning to trust and be cared for again and the struggles of becoming a parent when you weren't prepared.
Darker Than Black - This anime was so dark and esoteric it just made me overall sad. I mostly remember the blood and gore and the overall indifference the main character showed.
Speed Grapher - I like to group this anime with Darker Than Black because it was pretty much the same thing. Same plot, same concept. This one got me because of the child exploitation and the constant running away of something that was hard to hide from.
Deadman Wonderland
- Being framed for something seems incredibly harsh especially on a boy sentenced to life in a prison that's not like any other prison. Everyday is a struggle; being experimented on and having to fight to the death for the entertainment of civilians. How is this not sad?
Elfen Lied - Most overrated horror anime known to man kind but it does release the essence of unfairness and shock value to a "T". This anime made me feel helpless as I witnessed the terrors of the main characters.
Phantom: Requiem of Phantom
- Most underrated psychological anime out here? Imagine being taken from your everyday life because you were in the wrong place at the wrong time; witness to a super secret crime. Then imagine being brainwashed against your will as a soldier and not remembering the person you were before. All of your humanity stripped away. This anime gets more and more messed up as you watch it and I had to quit watching this anime for a few days to recover because it was traumatizing to me.
Wolf's Rain - When watching this I felt like I was in a dream. You know that feeling of trying to reach something but you can't quite reach it but you still try even though there is no hope of you reaching it? That was what this anime was in a nutshell. The journey of hope when all seemed lost. It was a beautiful allegory to me and I actually shed a few tears near the ending.
Rin: Daughters of Mnemosyne - So much gore and sadness this anime projected. When you thought it couldn't bring your morale down any further it does! It was difficult to watch at some parts because it was sickening and I wanted ever so much to stop it, but I couldn't intercept this anime. The ending made me want to throw up. Not out of sadness but out of... the blood? Torture? I'm pretty sure I became traumatized and refused to remember anything about the anime.
Gantz
-This anime also traumatized me. The first episode they weren't wasting anytime, BAM blood, guts and difficult decisions to be made. This anime scared the crap out of me and it made me depressed just watching a group of people being picked off one by one in a terrible manner. There was no humanity or mercy and it kind of just made me depressed for a while.
BTOOOM! - This one made me feel the same way at Gantz. They are kind if a similiar concept as well.
Pupa! - Where to begin. When I first watched this one I wasn't sure what to make of it. I mean, what in the hell did I watch? Maybe it was the incest or maybe it was the cannibalism, but it traumatized me to no end. I even watched the uncensored version like an idiot. What made this anime sad? The siblings took care of eachother no matter what happened. The fact that the sister turned into a monster and was constantly hungry and no matter how much she ate the hunger never ebbed.
Tengen Toppa Gurren Lagann
- Humans being controlled by an alien species and being forced to live under ground because the alien race wants to eradicate all humans. And then a true and large friendship between a boy and a man that never fades. The hope and determination everyone had to thrive once again on earth was just amazing to me. The hardships is what made it sad.
Brynhildr of the Darkeness - Have you ever been experimented on and made into something not human? No, I didn't think so.
Blade and Soul - The main character is so quiet and deals with things so smoothly. Her lack of expression depresses me because I felt like everyone could understand her easier if she were more expressive and it frustrated me that everyone misunderstood her.
One Week Friends - I've never met someone who had a memory problem. What made it sad was that you made a strong friendship within a week but then the next week the foundation you two built was destroyed and you had to start over again. Come on, everything about this plot made me sad!
School Days - Not only was I sad as I watched girls being played and girls being malicious towards one another, but I was mad at the main character for doing things that could have been easily avoided with a little moral and self control. This crap happens all of the time in real life, as far as fighting over a guy, not really sure of the ending. It was more like something you'd see on Lifetime Movie Network.
Aldnoah Zero
- Racism and war is never NOT sad.
Now and Then, Here and There
- Being taken away from your universe and inserted into another was what was completely messed up. Lack of food and water were what wars were over. Rape was the result of depopulation. Children were forced to become soldiers because there weren't enough men. Villages were pillaged because war called for more woman to give birth, more children to be trained as soldier and more men to start the cycle all over again.

Clannad After Story, I also agree that watching Clannad first is a must. I liked it but my husband didn't, he liked After Story a lot better though. I think both of them are really sweet in their own ways.

For me, I thought Clannad After Story was the saddest.
Lamune and Myself Yourself are a close second and third, though.
11eyes did have some pretty sad scenes too.

I saw kaguya hime no monogatari like a month or two ago, I swear I never cried river of tears since 5 centimeter per second... my dad was like why are crying so hard T_T

Naruto has definitely given me lots of sad moments, but I'm going to say SAO 2 (Mother's Rosario to be more specific). Saddest thing I've ever seen.

Keep in mind though I've never seen stuff like Clannad or Anohana, nor do I plan to (they just don't seem like I'd get through the first couple of episodes without dropping them).
