I look at the image, read the synopsis and then carefully tread the internet for reviews and production info, trying my best to avoid spoilers. Sometimes if I'm lucky I'll get a trailer for it too. That is all before I boot up the first episode. If the anime is produced solely by Toei Animation or Studio Perriot I will not watch it. If it has a certain theme I will not watch it. So my answer is yes and no.

If you're asking if it's some sort of moral failure to not do an elaborate cost benefit analysis of each item in a pastime. Then I have to say no. It's perfectly alright to judge YOUR time wasters by whatever criteria YOU find valid.

If it's has fanservice then I'll check amvs if it looks cool. If it does I'll give it a shot but if nots then no way. I like to say away from ecchi and animes with a lot of fanservice.

I usually go by the description or synopsis to see if its interesting. If it seems interesting, I might check it out.
Or, I will check something out a few episodes in if I hear from reviews that it's pretty good or some of my friends recommend it to me.
I think its okay to judge anime like this because you have no other real way to determine what you are going to watch, especially at the start of a new season when you have no real idea what each show is like or if its any good. Its reasonable to act like this to try and find something watch because, quite frankly, you have no other real way of doing to.

The only times I will judge an anime by it's cover is if it's evidently fan-service galore and/or ecchi. I can't stand that sort of thing.

This. Most of the time I'll read a synopsis even if I dislike the art direction/character design (like Tribe Cool Crew lol) but if the cover is just all women or harem-y I tend to pass on it.
