In terms of animation and visual styling I think Shaft, Bones and Madhouse do a great job. Of those three I think Shaft is the best at being innovative in its presentation of the material. Just look at mekakucity actors and ef: a tale of two cities.

I wouldn't really say the best, but I really like A-1 Pictures

Whenever it looks bad, you can often blame studios that do outsourced work.

Animation Quality:

Ufotable hands down. I haven't seen anything that has matched their animation quality and polish, they're on a whole other level. All their titles are absolutely beautiful and have extreme detail.

Faithfulness to the Source Material:

Out of everything that I've watched, probably Madhouse. They're usually very faithful to the source material with all their works. Ex: Hunter x Hunter, Black Lagoon, Hellsing, NGNL, just to name a few.

Toei is the worst studio imo. Out of the gazillion anime adaptation they've done since, what seems like, the 1700s, the only one they did a good job on was Slam Dunk. Most of their adaptation are lazily done and they rely on prime time timeslots ratings and the cash-cow factor rather than decent production quality. Just look at DB-Z-GT, One Piece, Toriko, Sailor Moon, Saint Seiya, Fist of the North Star, World Trigger, etc...

The best one has to be Ufotable. They, on the other hand, seem to have that Midas touch when it comes to adaptations.

Madhouse, Tatsunoko, and TNK because they put out some really underrated anime. (I.E. Casshern Sins, Kannazuki no Miko, and Karas)
