everything. but it really depends on what mood im in because if im sleepy and meet someone new all im thinking in my head is 'i need to go peeee ok' ok

The first impression is most important. I will decide then if I consider them a douche bag or not.

The first thing I typically notice is their demeanor. How they carry themselves or consider themselves. If they're confident and are loud, I will consider them a douche bag or a little stupid. If they are quiet and work hard, then I consider them cool and anchored - so to speak.

So yeah, the persons demeanor.

I guess the way they're dressed. I don't pay too much attention to the fine details - just try to get an idea of what type of statement (if any) they're trying to make by their attire.

Honestly, my hearing is more of my primary sense though - so usually I pay more attention to how they talk. Their tone, inflection, and general quality / sound of their voice.

My eyesight is too poor to notice fine features like their eyes, etc. Might be interesting if I could though.

When I first meet them there eyes (I have a thing for people with beautiful eyes) but as I get to know them I put them in the there cool or fuck of dickhead catergorie

I think it depends seeing as different things stand out on different people, but....generally just the body type. Height/weight type thing. Then perhaps hair length. I never notice eye colors. Still don't know the eye colors of people I've known for years >>; (being antisocial and awkward helps there I'm sure).

Shoulders and up, essentially, with a particular focus on faces. I usually don't pay much attention to details during a first meeting.

Their eyebrows - whether the shape is formed by hair growth or by their face wrinkles to a usual expression. Sometimes I'm even quick/keen enough to even notice how the folds in their eyelids contribute to the arching of their brows.
