They all sound super painful, but I'd say toothache. Although it's not completely the same thing, I suffer from Trigeminal Neuralgia, the worst of which I feel in my mouth, especially the teeth, and it's the sort of pain you just can't deal with. There's some pain you can work around as bad as it gets, but this is another level of pain.

ah, you poor thing, that really sucks. TGN is a bad one to deal with. is there a nerve being pinched or something that causes the neuralgia?

First of all, sorry about the delay in responding back. As for the causes, the doctors seem unsure as to why TN happens. The general gist I get is the nerves...misfire? The slightest thing like a gust of cold wind, brushing teeth, even drinking, eating or smiling can trigger it once it starts. It's more of a thing I need to manage now, but I take each day as it comes. Thanks.

For me it'd be toothache because I'm cripplingly dental-phobic. I find pain in my teeth far more traumatizing than any other damage.

I'd say toothache. I get so irritated when I have toothaches because it's like a small prick that constantly bothers you, and there's nothing to do about it right then and there. Especially when I want to eat or something, I get angry. I still remember the times I've had it and I pretty much felt like ripping the tooth out of my mouth.

I'd say a lot of these is down to your own personal pain threshold, though from my own experience it would be under stomach pain.
suffering with UC (Ulcerative Colitis) and IBS...my god when they decide to flare up the pain can utterly leave you bed ridden, since in most people they tend to have a knock on effect throughout the digestive tract.(Nausea, vomiting, cramps/spasms,heartburn and headaches to name but a few)

No definitive way to say which is worse end of the day, as there is always extremes from each.

I'm gonna go with muscle because... well... We're basically all muscle. At least with all those other aches you still kind of can move around.

Ear pain sounds horrible in general. Ears are kinda bad enough since people can get pimples in their ears

Well I just accidentally poured boiling water on my fingers, so my current choice would be burning your fingertips
