Art style, premise, and merchandise. The last one is probably at least partly responsible for half of the stuff I watch due to a figure release that catches my eye.

The very first thing? The art for sure. While I do have my favourite genres, I can be tempted to at least look further into anything as long as the art style and character designs appeal to me.

The first real test, though, is a short summary of what it's actually about. There have been quite a few series that I've loved the look of but don't even watch a single episode of because, for example, I read the synopsis and find out it's just another run of the mill harem show.

Unless I've read the manga (or VN or LN), I always read a review before I watch anything.

That's not to say that I always follow what the review says. I don't care about the reviewer's conclusion, instead I'm looking for:
a) any clue about whether or not it comes to a satisfying conclusion,
b) whether or not it's especially heartbreaking (I generally don't like sad stories), and
c) is it a genre that I generally enjoy (romance, comedy, slice-of-life, harem, or an extremely well told adventure).

I generally look at the genre and synopsis.
And if something is described as being really weird, I'll often give it a try.

The brief summary gets me, then the art. I don't like super-deformed styles much or ridiculous echhi fan-service-y storylines. Also no patience for air-headed moe characters and dull as brick love-interests.
