I was gonna say maybe they were already sick and didn't know it yet, they they got wet in the rain and it just kicked in. It's possible .... I don't know the science stuff ....

This so this, everyone knows population density is the leading cause of the spread and mutating of various illness. It's a wonder the entirety of the Japanese Islands isn't home to some super bug ya know.

Instantly getting sick from getting rained on is a common plot device in a lot of media, not just anime. Sitcoms and American cartoons do it all the time. But as others have mentioned, there are some real life factors that result in the need for those surgical masks. If you walk to work or ride the train wet and cold every day for weeks, passing hundreds of people, wading and waiting in crowds...yeah, that'd have some affect.

Even in Canada, I sit as far from people as possible on the bus.
And our public transit and pedestrian traffic isn't nearly as dense.
