When you can't understand someone because of their accent.

Have you ever been in this situation?

What would you do?

Pantomime.
Body language is the best because there's every chance they have as much difficulty interpreting an english speaking accent.

I will ask that they repeat themselves one time, and if I can understand a little bit of the sentence, I will make an educated guess. If their accent is so thick that I cannot get anything, I might ask them to write it down/type it on a phone.

This question is highly context reliant

It happened/happens with me many times considering that I live in a country with diversity of languages so I usually ask the person to repeat if I still don't get it I'd smile and nod

Smile and nod boys , smile and nod. That has worked for me thus far I have no plans to abandon a plan that is working.

Since I kinda bounce back and forth between my country and another I run into this problem all the time when speaking Spanish since i'm only at an intermediate level, especially if someone has an accent from a region I'm not familiar with. Yeah repeat or speak slowly usually does the trick, if not have them explain the word or phrase in a sentence.
