I cant imagine people who would want to torture themselfs in a much higher difficulty in a video game. Any game known to be hard and made by the developers in that way.
Only to make the game harder but how hard is too hard, what makes it hard, how much harder can it get on higher levels?
So far each game is game is different.
Games from metroid, zelda, dark souls, dead space. Resistance fall of man. Mega man x. Rpgs.
They all do it differently
From enemies, stages, big enemies & bosses. They all can suck and make it hard
But what is it?
Hard, Very hard, legendary, nightmare, impossible?
What does it mean?

More enemies?

Enemies/Bosses do more damage?

Little to no ammo/hp?

Down to one life?

Weak guns?

Little to no check points?

Smaller hp?

Weak armor?

Some people take the games games difficulty beyond by not getting armor (megaman x or metroid)
Use a item to make it harder(star fox zero's falco amiibo for the black ship. Zelda twilight princess hd hero mode & ganon amiibo, dark souls ring to take more damage?)
Or when a game's boss is so hard that not even the devs can beat it?
Why is it kept in the game?

There isn't a single game out there that is hard . A hard game would indicate the AI can think, adapt to the player's tactics and change accordingly, come up with unique attack patterns on it's own on the fly . A damage nerf to the player or a bigger hp bar for the boss does not a difficult fight make, just a longer more boring one.

As for why developers think it does and why they keep it in the games that are being made. Lack of intellect to make an actual hard game is the only reason.

Difficulty in games is such an odd topic. As Ranwolf says, the only "true" difficulty would come in the form of an AI that could actually think, learn and act as a human but still within the reality of the game's rules. Games have played around with the concept of a learning or predictive AI before and, while it does often lead to more difficulty, it never really feels right.. it often just feels cheap or broken. Take something like a fighting game AI that reads your inputs instead of responding to what you are actually doing. Is it hard? Sure, but it's not fun and it's also not indicative of quality design. Developers often do have to pull back a little on AI because of that. If the AI is "too good" then the game just isn't fun any more and it simply becomes a race to find ways to exploit things before you get too frustrated or bored to continue. If AI could be made that would accurately model both the strengths and weaknesses of a human player then you'd get true difficulty, but we're just not there yet.

Put simply, yeah, it's very easy to make a game hard through a variety of means but you need to achieve a level of fun and fair as well. This is one reason why I feel games like the Souls series and, a personal favourite, Ninja Gaiden on the Xbox really work. They are hard, sometimes very hard, but they feel fair. You, as the player, are given adequate tools to deal with everything they game throw at you without just giving the enemies totally impossible attacks or bloated HP amounts. Both the Souls games and NG allow you to literally play without upgrading/levelling and potentially without even getting hit.. provided you are good enough and understand the mechanics well enough.

Ranwolf is right but then again, it's one's personal opinion. Here's a little rundown in hard mode or higher (what i find hard or such):

- You take less damage (any game that has hard difficulty or higher), that's something i can agree.
- Your special abilities can't be used ("listening" ability from The Last of Us, for example). Well, i suppose it's all right and get use to that as if you didn't have that at all.
- Enemies respawn. No, not Mega Man. It's okay in that. I'm talking about Killzone 2. Play the Bridge level on Elite, reach the end of the bridge, kill some of them, turn around and back and: tada! They're back!

Here's one worthy mention:

Those annoying defeat bosses without taking damage AND clear it on hard or higher-Trophies/Achievements in Metal Gear Rising: Revengeance. Yeah, i know this may not belong here but just wanted to say that. As if the Trophies/Achievements were not hard enough to get... And the overall difficulty for all those is 10/10. And i still i have to get them. Damn! But as a Trophy Collector...

Blame games that test nothing but twitch reflexes and rapid responses. Games like the Soul Series are really nothing more then glorified puzzle solvers. Ya not trying to out manoeuvre or out fight the hordes of enemies and boss in Dark Souls , ya trying to out think them. Critical thinking and logic are in short supply these days though.

I think online games are the hardest against other human beings, that seems to be the hardest enemy in video games right now.

I game is too hard when the "hard" part about the game is not the AI itself but rather you're handicapped or the AI are given extra things/stat bonuses. This is called poor AI design.

hard games often throw you against enemies that are level 70+ while you are level 10..

there's no check point.. or the checkpoint is so far out that you have to start the level again.

add salt to injury they loading is annoying long! i'm looking at your Bloodborne!!!

over time it will become of a hassle

games like uncharted and the tlou on the most hard difficult were never a problem for me .. thanks to generous checkpoints where you can restart right at the start of the area.. instead of going way back

A difficult game is all dependent on the player. Some things are simply easier for others. I agree with what Ranwolf said to an extent, but I think his statement would just mean that difficulty isn't a thing in any aspect. I simply believe duration it takes to achieve something is a factor in difficulty.

Really just depends on you. If you're not good with puzzles into a game, that is gonna be difficult for you but a breeze for someone else.

It depends on the genre for me and really it's a preference for everyone. When a game is too hard, it's no longer fun simple as that for me. I tried bioshock infinite on hardest difficulty and that was just meh. Then again I don't like that game. I do like to play on hard difficulties when I can.
