I watched a new title yesterday on my XBox One and added it to my queue. Today I wanted to continue watching, but the title was not there. Not only that, it's missing from every platform (XBox, PS4, Android, iPad). Yet when I go to Crunchyroll.com, there it is! It's also missing from my history on all platform apps, except the actual site. Also cannot be found with an in app search.

Why could I watch it one day and can't find it the next? Why is the only way to watch through a web browser?

Ok, apparently there was an update or something. I made some changes to my profile settings and they appear now as they did yesterday.

Really pissed off that changes were made to my profile without my knowledge or consent as part of a so-called update. No wonder I let my premium membership go...

We do not make changes to profiles unless there is something on there that violates our terms and conditions, e.g. pornographic images, obscene messages, etc.
Looking at your profile, there is some broken image code in there. Is that what you are talking about?
