The USA is a giant 'F YOU' to the conventional politics of the rest of the planet Earth. The Founding Fathers were extremely paranoid about giving the federal government any sort of authority and put many limits and contingencies on what authority they gave the government, often pitting the government against itself by design. The result is that American politics and government institutions seem almost indecipherable to non-Americans and Americans have mutual problem understanding the politics of other countries.

I would think a US monarchy would be close to the Holy Roman Empire. The likely starting point would have been George Washington accepting the first Kingship of the United States which would have been wholly antithetical to his character but perhaps he accepts on the grounds of stability. I don't see Washington or the Continental Congress or Constitutional Committee countenancing a hereditary monarchy, but allowing an elective monarchy. As people have pointed out, the Polish-Lithuanian Commonwealth was an elected monarchy, as was technically the Holy Roman Empire although with a very limited franchise, as was the kingship of England prior to the Norman conquest, so elected monarchies were not unheard of.

But largely it is just a title change. Likely the Congress would have been loathe to give the monarchy any teeth so it would just be the Presidency, but with a different title.

Not a huge fan of monarchies. Being in a position of power solely based on your lineage seems wrong to me.
That being said most European monarchies nowadays are little more than state figure heads and have little actual power anyway which is quite a bit different than what many of us are probably referring to.
The closest thing to absolute monarchies I can think of would be many of the Middle Eastern royal families who have near absolute control over their subjects, which really seems quite unfair.

A lot more would get done. Dictators are the most efficient forms of government, but they're prone to giant screw-ups without any checks and balances on their power.
