Toradora - Classic odd couple rom-com. The first episode gets a little over the top, but it settles down into a (mostly) surprisingly grounded story.

Tsukigakirei - Recent, but extraordinarily realistic take on middle schoolers falling in love.

My Love Story - Sweet and earnest.

Our Love Has Always Been 10 Centimeters Apart - Too many weird choices, on and off screen. (Mom is obviously a psychopath)

Re:Life - Decent anime, but then read the manga (on CR) which is when the real twists start to show up.

Your Lie in April - By the end you'll either be sobbing or thinking that someone was a bit of a dick.

Lots of good recommendations so far.

Sweet Blue Flowers is excellent. Lovely, intimate take on yuri with some genuine humour and really likable characters.

I also have to shout out Tsuredure Children. It may be more way more comedy than captial R Romance but it really gets right to the heart of romance in anime with a whole bunch of pairings that sometimes feel more genuine and experience more progress than you'll see in a lot of full length shows.

If you're wanting a more messy and dramatic side to romance, then Scum's Wish and Love and Lies (Koi To Uso) are both excellent. Both may make you want to throw something at the screen at times but it's because they actually have characters that interest you, grab you and pull you in. You just don't get that with the flat, cliched characters you see in most romance series.

I'll also throw Citrus into this list as well even though the anime is still fairly early. Some of the subject matter in it makes some people very uncomfortable, but it's got great characters and story that smartly threads the line between it's dramatic and comedic elements.

Well if you want a show where the MC takes the first step in the wrong direction of a relationship, then I recommend my personal favorite romance anime, School Days. Oh and White Album 2, honestly this one is better than School Days, and actually the premise is kinda the same now that I think about it but WA2 isn't over the top like SD.

Kare Kano, Nana, and Nodame Cantabile are the only series I acknowledge as truly great romance anime. There are others I like, a handful which have already been mentioned, but the 3 I wish to share, in my opinion, seem to depict romantic/sexual relationships and the many themes of love the most realistically and maturely, and aren't confined or built on the typical anime tropes.

Ai Yori Aoshi. You will need to read the manga to finish the story though and the character designs are kinda dated but still well worth it.

For some reason I've always enjoyed the romance in: Zero No Tsukaima

Boy summoned to a fantasy world as the familiar of a mage who comes from a royal family and who is attending a prestigious mages academy. It's fun to watch them go from arguing with one another to eventually warming up to one another through their hardships.

The slow build in relationship for Ranma 1/2 has always been enjoyable to watch. In amongst all the chaos, you get these snippets as the romance develops, its sickeningly endearing.

I really like Convenience Store Boyfriends. It's a bit predictable and sappy, but it has some good back story and a little twist to it.

I will also second MMO Junkie. It is something different from the typical high school romance, and the characters are very endearing.

I even like Blue Spring Ride. The ending leaves a bit to be desired, but overall it's a beautiful anime worth the watch.
